Strategic Partnership Formation

Crossroads Coalition has participated in numerous coalition and collaborative efforts to address the health needs of South Suburban Cook County residents. These collaborative partners include businesses, faith-based organizations, community-based organizations, state agencies, and local governments. The aim of these collaborative partnerships is to combine knowledge, skills, capabilities, assets, and resources to effectively advocate for policies and programs; facilitate/coordinate health and social services; and empower people with access to quality healthcare. Crossroads Coalition’s approach to collaboration with these diverse groups is through shared information, planning, and program development.

 

Community Resourcing

Crossroads Coalition is a driving force and lead agency for securing funding to address the health needs of targeted communities. Since its founding, Crossroads Coalition has been a conduit for accessing thousands of dollars in foundation and government grant funding related to health disparities. Additionally, Crossroads Coalition links communities to vital health information, materials, and needed health/social services.

 

Community Capacity Building

Crossroads Coalition has an extensive history of providing technical support and training to build the capacity of organizations. Capacity building activities include skills training, train-the-trainer programs, organizational development, program development, and network building. As a part of its capacity building initiative, Crossroads Coalition co-organized and provided capacity building support to the Southland Ministerial Health Network, with the goal of developing an infrastructure for/in the faith community to engage in health promotion activities.

 

Outcome-based Programming

Crossroads Coalition develops and implements programs that are designed to produce impacts/benefits/changes to individuals and communities. The outcomes sought by Crossroads Coalition programs are:

  • Increased knowledge
  • Changes in attitudes and values
  • Increased skills
  • Modified behaviors
  • Improved conditions
  • Improved quality of life
  • Increased service delivery effectiveness
 All programs developed by Crossroads Coalition operates from a logic model that links inputs and services to output, that result in the desired short, medium and long-term outcomes.
