Faith Communities
Tackle Chronic Disease

Through a grant from Cook County Department of Public Health (CCDPH), Communities Putting Prevention to Work Grant (CPPW), Crossroads Coalition is using its existing partnerships with faith communities to develop the infrastructure to offer chronic disease self-management programs within eight targeted communities in South Suburban Cook County, Illinois. Nine faith community partners are offering "Take Charge of Your Health" workshops at their faith sites. The Take Charge of Your Health workshops are a skill-building workshop series that helps people with one or more chronic conditions (e.g. diabetes, chronic pain, hypertension, cancer, arthritis, etc.) learn to manage their condition and their life. Nineteen members from partner faith communities have received their certification as Take Charge of Your Health Peer Leaders. This certification comes through Age Options and Stanford University.
